Our practice is committed to providing you with safe, high-quality medical treatment. We understand that you may feel anxious about visiting a physician. We are sensitive to unique needs of each patient and it is our goal to make you feel comfortable visiting our practice while providing the best care possible.

ABOUT YOUR VISIT
Preparation for your appointment

During your first visit, we will review new patient medical history forms. Then Dr. Golden will meet with you to discuss your medical and/or substance use history, help create an individual treatment plan, and answer any questions you may have.

 

Remember to bring the following with you to your first appointment

 

  • Your completed new patient forms below

  • List of medications

  • Your insurance card

  • Your Driver's License or picture i.d.
